View Full Version : Case study - persistent pain/swelling after sprain

This question is probably directed more towards any physios/health professionals on the forum, but also to anyone having severe ankle sprains....
A friend of mine was asking my opinion of a bad ankle sprain he said he suffered 6 weeks ago that was still causing pain. On observation, there was a large mass of swelling on the anterolateral aspect of the foot, probably a couple of cms distal to the lateral malleolus. WB did not reproduce pain, but palpation of the anterolateral foot did. Anterior drawer test was painful, but not lax. What worried me most about the injury was the persistence of the pain & swelling, and that RSC eversion elicited pain - that it possibly related more to peroneal tendon or possible associated bony injury. As such, advised him to see a doctor/physio to get it seen to (by someone qualified!) and possibly get an XR/scan. XR showed no structural abnormality, and was diagnosed with a severe ankle sprain.
I was wondering if others have similar cases of patients with sprains presenting with persistent swelling and pain this late after onset? The amount of swelling still there suggested to me there may be continuing active inflammation that is aggravated by WB/walking (he's not really the athletic type), rather than a healing lateral ligament sprain. Any thoughts?
