I have been suffering for years with this pain. I play rugby league and constantly keep injuring my patellar. I had a scan a few years ago and the Dr said the inside of my kneecaps were split on both knees from moving up the wrong track. I have got a lot better from then, I have a history of bad ankles as well. My ankles roll in. I have been able to come back very fast after spraining my ankle since i was a kid, but it got me thinking about my mechanics and that pain works up. My ankles where bad and that may cause my knees to play up and my knees make my hip play up. I now wear inserts in my shoes to correct my ankles, which hopefully fix my knees. My right leg use to be painful like the left and use to click. The muscle above the knee cap was always tight, My right leg has torn the quad at the insertion and as a result my patellar is alot better. But my left is still a problem.
Also have been talking with a number of mates and studying walking of other people. As i have had this imbalance the Vastus medialis is alot bigger than the Vastus lateralis. I am now trying to get the Vastus lateralis bigger to try and fix my knee.
My QUESTION is how could i target this muscle and how long do you think it would take as i want to play footy? Also could i accentuate my ankle inserts by padding the inside so it cause my roll to go out to help the muscle stimulate?

Sounds unusual as normally when you have swelling on the knee it is the vastus medialis which shrinks. This should be muscular much lower than lateralis.
It's pretty hard to give a prognosis over the internet. You should book in to see a physio or a doctor to have them look at your knees and operation reports and work out whether it is realistic to play football.

i saw a specialist and had a MRI i have some cartilage tears and on the outside of the left knee cap i have a tear at the back of the patellar tendon. I have to do physio if there is no improvement the dr will split the good tendon to get through to the back and cut out the tear and scare tissue.

It is hard to make an accurate diagnosis without seeing your knee or the scans, but if the experts looking after you think the main pain is coming from the patellar tendon, I would hold off from surgery as long as possible. Surgery generally isn't the best treatment for this condition. Moderate loading and eccentric strengthening are worth trying, maybe nitrate patches.

I have started physio i am doing essentric exercise to lengthen the tendon by doing the negative motion with my bad knee and pushing back to starting with the good leg icing after exercise. I also have to do bike riding to strengthen the quads. I have felt good but will give it time before i start running which will be the test. Also have been taking glucosomine.
