I have a 19 year old rugby playing patient, who last year experienced a sudden onset of right hip pain. He can't remember doing anything to it and has no history of hip injuries. Since the onset he has developed SI pain and more recently chronic painful muscle spasms in his quads.
On examination he has very limited movement in his pelvis and has a leg length discrepancy of just over an inch.
He has been resting from playing rugby this season but this is not helping, core stability exercises, ultrasound, acupuncture or manual techniques have been of no use. He is on NSAIDS.
Psoas bursitis has been queried, but we are unsure as to whether this is a correct diagnosis due to him not responding to treatment and the problem gradually getting worse. He is awaiting scans.
