I have intense pain around the top of my fib. on my R leg ONLY after i run for >45 min. This has been going on for 4 yrs. I've tried strengthing my glute min., custom orthodics, stretching, RICE, etc. NO improvement. Very limiting injury, and no one has a clue what is wrong.

Someone else out there must be able to help. Please!

Pain starts around the top of the fib., then radiates down to my foot and up to siatic nerve. pain is unbearable after 50. min. of continuous running, but there is no pain after resting for ~30min. Next day I'm fine and i can run for 45 min. again.

My guess would be iliotibial band friction syndrome, based on:

(1) pain only after continuous running for long period
(2) outer (lateral) side of leg

Other factors that would confirm this diagnosis:
Downhill running would be worse
Playing tennis/basketball etc would be better
Walking with a stiff-legged gait would be the most comfortable thing to do after the pain comes on

If you think you have this, you can try a cortisone/local injection to see whether this blocks the pain for a while (however, you'll need a doctor who knows how to diagnose running injuries).

Other possibilities: ?sciatic nerve entrapment, lateral compartment syndrome.
